Claude Code 新增 AGENTS.md 支持并保留 CLAUDE.md 优先级

Claude Code 现已支持读取 AGENTS.md，默认优先加载文件夹内或上层的 CLAUDE.md，CLAUDE.local.md 同样生效。用户在 /config 中将 project instructions 设置为 claude-md-and-agents-md，即可按顺序同时加载两个文件。
